| New Summary Report  
The new CHADIS Summary Report offers clinical and questionnaire data in three viewable / printable views: **Summary (1)**, **Detail (2)** and **Interactive (3)** (numbers in red denote placement on the screen shot below). You can access the various views by clicking the name of the view in [brackets] at the top of any screen.

| (1)  **Summary View**<br>This view is a single page that lists parental concerns for the visit, tabulates all the questionnaires taken and summarizes the Provisional Results (scored per the manual for each test) and Alert levels (a visual triage system developed by our team). | (2)  **Detail View**<br>This view contains the Summary view table plus a detailed review of scores and interpretations for all questionnaires taken, listed first alphabetically and then by Area of Functioning. This version prints on multiple pages. | (3)  **Interactive View**<br>This is the view that existed prior to the new report and is still available. It enables entering clinical notes and is the only view that provides access to decision-support features (e-chapters, resources and evidence-based templates). This version prints in the old format by clicking on the printer icon at the top right of the screen. |

The Summary table, in all views, allows users to select to "Preview" or "Include" the actual questionnaire responses in the report **(8)** by clicking the appropriate link in the same row as the questionnaire name.

At the bottom of the table **(9)** is a link for changing to a "copy-and-paste-friendly version," which accurately formats for pasting into EMRs.

## Description of the Table Columns

**(4)  Alert Level**  
This column provides a visual scalar of alerts to highlight the potentially most important results. The level of each alert was determined by our pediatric staff and was not reviewed or endorsed by any other professional entity. Clinicians should review all the data and decide what needs to be addressed.

**(5)  Questionnaire:**  
This column lists the name of the questionnaire completed. Graphical display of results from Vanderbilt questionnaires are available by clicking the icon in the questionnaire column that looks like a small graph.

**(6)  Completed / Respondent**  
These columns state the date the questionnaire was completed and the respondent who provided the information. This information may also be needed for 96110 billing.

**(7)  Provisional Results:**  
Results in CHADIS can have one or several severity levels depending on the questionnaire and the responses given.

- **Pass / Fail:** These columns list the number of pass or fail subscores for this questionnaire. The number "1" may mean that there was only 1 result for the entire questionnaire and that it was in the abnormal range or that 1 subscore was abnormal in a tool for which there are several subscores, in which case the number of other passes will be listed.
- **Disorder / Problem / Variation / Challenge:** These columns list the number of provisional diagnoses at each level of severity for this questionnaire. Descriptions of the standard meaning of each level are available by clicking on the column label.

**(8)  Responses**  
This column allows users to select to "Preview" or "Include" the actual questionnaire responses in the report by clicking the appropriate link in the same row as the questionnaire name.
